Changes to the way healthcare providers are reimbursed for their services require them to evaluate every decision as patients move through various levels of care. This is the primary responsibility of the Clinical Integration Team at Parkview Health. The most critical decisions are made when facing skilled nursing care options, with the goal of having the right patient at the right place at the right time. A driving consideration is allowing the patient to return home as soon as possible with alternative care verses an expensive skilled nursing facility. Skilled nursing facilities increase risk to patients due to infection risks, expenses, and added stress, which impedes recovery due to separation from family.
